A Health Home is not a place; it is a group of health care and service providers working together to make sure your child gets the care and services needed to stay healthy.
Health Homes are a part of the NYS Medicaid program and available to Medicaid-eligible children who have health/mental health concerns and conditions that may benefit from the careful planning and service coordination provided by Care Management. The goal is to promote optimal care for your child.
Green Chimneys is proud to be a Health Home Care Management Agency, helping to coordinate the full range of services your child may need. This may mean fewer trips to the emergency room or less time spent in the hospital. It could mean getting regular care and services from doctors and providers. Or it may be finding a safe place to live and a way to get to medical appointments.
Once you enroll in a Health Home, a Care Manager will work with you to develop a plan of care for your child. Specific services may include:

Enrolling in the Health Home program is your choice. There is no cost to you. If you think a Health Home may be helpful to you and your child, enrolling is easy. Our Care Managers will help you complete the necessary paperwork and explain everything to you in detail.
Our services are individualized to support the unique strengths and needs of each child. Our collaborative relationships with providers throughout the region allow us to ensure that your child will have access to the services he /she needs most.

A school that partners children with animals for educational and therapeutic activities is an ideal home for Nutmeg, a Nubian goat donated by a family who kept her as a therapy animal for a child with special needs. Nutmeg arrived at Green Chimneys as an energetic one-year-old. Although she had lived solely with people, she immediately adapted to her new goat peers at the farm. At the same time, she presents a challenge to students: goats possess a strong will so taking a walk is not always a simple task.
