Case Management
Green Chimneys Community Based Services are inherently preventive in nature and designed to keep children in their community and with their families, avoiding residential placement whenever possible. Resources for parents and families include access to mental health assessments, social supports, and service coordination to help families provide the best care for their children.
An experienced staff of certified social workers, case managers, program managers, administrators and interns work closely with youth and families to provide individualized services, skill-building tools, and resources to support positive individual and familial development, self-reliance and healthy communication.
Case Management includes
- Health Homes Care Management
Full range of service coordination through New York State Medicaid for children with health/mental health concerns
- Intensive Service Coordination
Comprehensive intervention to help youth stabilize their life, foster pro-social behavior, and strengthen family relationships
- Mandated Preventive Services
Resources to stabilize a child’s home environment and prevent need for foster care placement, in coordination with Putnam County Department of Social Services
